[Editor's note: Note to all people who have interaction with the healthcare system. I had my bone marrow biopsy and it came up clean -- no infection in the marrow. That was pretty much what the hematologist, Dr. Brandon McMahon, could bring to the table. It is not a blood production problem.
That left the ball in the court of the Infectious Disease doc, Michael P. Angarone, who struck me as thorough and sharp at our first meeting but since then he has disappeared. Meanwhile the low grade fevers continue indicating some kind of infection. I was and am getting very frustrated. Angarone sent me an email responding to my email stating that:
"Mr. May
If the fevers and sweats continue we may have to perform a CT of the chest and abdomen to make sure there are no hidden sources of infection.
I apologize for not responding sooner, I am seeing patients in the hospital and it has been a busy week. I will write again later today to see how you are doing."
I did not hear back from him and today, Dr. Eduardo Cremer (my kidney doc) and I discussed the situation in the clinic. Cremer asked me what Angarone is waiting for.
Precisely, what is he waiting for? I have been on oxygen for 35 days now. My hemoglobin is still very low and I am quite anemic. If I walk from my computer to my kitchen, I am out of breath and worn out.
The persistence of the low grade fevers is not in and of itself a terrible thing, but the infection it indicates may be the source of the problem with the hemoglobin. I am what they call "epo resistant," that is resistant to the drug that made Amgen rich, the miracle drug Epogen (sometimes called Procrit). That condition can go away, but when? Dr. Mark Leischner, Dr. Cremer's colleague, told me today that he had an epo resistant patient and poof, it went away after five months. That is great to hear, but at about 58 days now, I don't think I can endure for three more months.
Yesterday, I actively started making plans to go to Rochester, MN to the Mayo Clinic on the 27th if things don't start to change. But after talking to Dr. Cremer and getting encouragement from him, I called Dr. John Flaherty who heads up the Infectious Disease department at NMH.
He called me back about 12:45pm today. I explained the situation calmly and intelligently to Dr. Flaherty who did call me a few hours after I called to ask for him. We talked for twenty minutes. He acted very quickly. First, I am already scheduled for the CT scan this Friday at 2pm, the purpose of which is to check the abdomen and pelvis with intravenous and oral contrast (I have to do a steroid prep starting 13 hours earlier) to find out if there is a hidden infection such as an abscess. Man that was fast! And Dr. Flaherty did not defend Dr. Angarone in this instance.
Right now, I am pumped. They were letting me die here and I said, no way, I am not going to let this thing drift into the abyss. I have too many people to destroy in the report. :-) Where's your sense of humor, bub?
On top of that, Dr. Flaherty has set me up with another Infectious Disease doc at NMH named Dr. Noskin and that appointment is for 3pm on the 24th, a day with many tech events.
Moral of story. You have to take matters into your own hands as a patient. You, the patient, have to be driving that bus or it will drive you right over a cliff.]

Deadline for preliminary application to State for funding is 2/18. Submission deadline to BTOP is 3/15.
Actually, quite interesting. Will be using senior public housing buildings as the "hubs" for the development of Internet-supported senior support networks.

At 9:26 minutes Janet Tavakoli discusses Wall Street's pre-meltdown actions:
Poking through the wreckage, many experts believe the root cause was a perfect storm: a monsoon of gullibility colliding with a tidal wave of greed.
"This was a massive Ponzi scheme. And it's the biggest crime against the American economy in our lifetimes, in fact, ever," analyst Janet Tavakoli explained.
Tavakoli is an analyst specializing in derivatives, the exotic financial instruments at the heart of the meltdown. She argues that the bad mortgage loans that fueled the crisis were repackaged by investment banks, sliced into increasingly complex derivatives and resold to other investors, even though the underlying mortgages were often virtually worthless.
"You had various traders buying each others' products to artificially keep the prices up so that the bubble didn't collapse," she told Safer.
Not only that. But the mortgage derivatives being traded were so mind-numbingly complicated, nobody understood them fully. Certainly not the pigeons: the buyers at banks, mutual funds, pension funds and insurance companies who wound up holding a bag full of worthless paper.
"But these guys are smart guys. They're all graduates of the finest business school, correct?" Safer asked.
"Yes. If they were gullible, they're sophisticated investors. So they can't really go back to the investment banks that sold them this product and say, 'We've been had.' Because they held themselves out to be experts in these kinds of securities," Tavakoli replied.
All of which proves that whether you're on Wall Street or Main Street, brain power is no defense against con men. In fact, smart guys may be the biggest suckers of all.

Also from the interview: "'We wouldn't want to live in a world where we couldn't be conned because in effect we would then be living in a world where we mistrusted or refused to trust anyone. So this is the price we pay,' [Ricky] Jay said."
I agree with Mr. Jay. Who would want to live in a world like that? But equally, who would want to live in a world where trusting hardworking taxpayers are unprotected by government so they become the easy prey of exploiters? That's why we have laws. That way, we can remedy wrongs and make criminals pay the price. It's time to put that theory into practice.

I still owe you a few Information Superglacier(TM) stories, but the Google story broke out of the gate on its own.
Below is the text off of a PR Newswire release for Ontario County telegraphing its punch to Google before Ontario County gets lost in the mad rush.
The fiber system is past shovel ready and I hope to have a few smaller, less well known companies to talk about in the promised articles in a few weeks.
It looks like we will have one or more 4G systems using the fiber. The Google opportunity is the second FTTH project discussed in as many months. It turns out building a vendor neutral backbone with open access which many lampooned at each step of the way might be a solid economic development engine.
One irony, our are connectivity was considered to good to qualify for the stimulus funds, even if the non discrimination and neutrality were the core principles of the network all the way back to the grass roots discussions back in 2003. Cable companies and LECs have joined in the project to extend faster service into new territories. The NY Rural digital divide is only a few miles south of where I am.

With 180-mile fiber ring, County has what it takes to be one of Google's
ultra high-speed network trial locations
CANANDAIGUA, N.Y., Feb. 11 /PRNewswire/ -- A day after Google's blog
that rocked the blogosphere, Axcess Ontario is announcing it wants Ontario
County, N.Y., to serve as a Google test site.
Axcess Ontario, the public-benefit corporation overseeing the
development of the County's 180-mile, open-access fiber ring, started the
official application process on behalf of the county government yesterday
and hopes to complete it early next week.
In a blogpost yesterday morning, Google announced it was seeking trial
locations for ultra-fast broadband networks. The company issued a Request
for Information (RFI) to communities nationwide seeking test sites for an
experiment to make Internet access better, faster and less expensive for
everyone. Its networks will deliver Internet speeds more than 100 times
faster than what most Americans have access to today over 1 gigabit per
second, fiber-to-the-home connections.
What Google seeks to accomplish is exactly what the Ontario County
fiber optic ring was built for, said Axcess Ontario President and CEO Ed
Hemminger.
"Ontario County should be at the top of Google's list of potential test
sites -- we've been working on this project for seven years," Hemminger
said. "And with the 180 miles of fiber scheduled for completion by the end
of the year, Ontario County is perfectly positioned for Google's
experiment."
A rural/suburban community outside Rochester, N.Y., Ontario County has
committed $12 million to its fiber ring project. Approximately 60 miles of
the fiber ring are complete and currently being used by service providers
such as Verizon Wireless, tw telecom and Finger Lakes Technology Group to
provide cost-effective telecommunications to businesses along the 60 miles.
The remainder of the ring will be completed this year. This means Ontario
County actually has the infrastructure to do what Google wants to do -- and
quickly.
"Our community had the vision, knowledge and the foresight to invest in
a fiber infrastructure that is critical to American innovation and economic
growth," Hemminger said. "The fiber ring ensures our community will never
be left behind in the global economy, and this week we can see with the
Google experiment that Ontario County, N.Y., may, in fact, have an
opportunity to lead the way."

Ron - Yes, skiing and snowboarding can be dangerous.
Every year in North America a couple dozen skiers and snowboarders die from accidents, avalanches, and smothering. Sonny Bono died two days before I was skiing in Heavenly by hitting a tree in 1998. So did one of Bobby Kennedy's sons, Michael, in 1997 in Aspen. And, John McWethy, the ABC news correspondent who attended DePauw University with me died in an accident at Keystone in 2008 when I was on the same run (see http://www.usatoday.com/life/people/2008-02-07-mcwethy-obit_N.htm). He apparently caught an edge on his ski, lost control, and hit a tree.
A human will lose in a collision with a tree every time.
Many skiers ski out of bounds where avalanche control by the ski areas and officials are not reducing the risk of avalanches by setting off explosions to shake unstable snow layers loose. If they are skiing and are caught by an avalanche, the result is frequently death by either suffocation as you cannot dig out of the snow as it freezes almost instantly to a consistency that is like hardened concrete or due to the injuries incurred when you are run over by the moving mountain of snow. (In an avalanche, the snow moves at speeds up to and sometimes exceeding 200 miles per hour, which is why it freezes up - the friction of the fast movement melts the snow crystals and the cold then freezes it when it stops moving.)
The other main cause of death is when you ski or snowboard into a tree well (a hole surrounding trees, especially pine trees, as the snow that falls lands on the branches). That snow is loose and soft and if you fall into such a hole, you can find yourself buried in the snow and smother. If you are on a snowboard, you will generally fall into a deep tree well upside down and since your board is attached to your feet, you will hang with your head buried in the snow and smothering may occur.) One should never snowboard alone as you may need help to get out of a tree well.
There are also folks who have died jumping off of cliffs or other things and break their necks or have head trauma. Obviously, this is why many wear helmets (I always do when I ski and board out west.
As to the luge, check out the Chicago Tribune's sport section today (I think it was) and they have a graphic of the luge course on which there are sharp turns and speed can go up to 90 miles per hour. While they wear helmets, a crash at that speed can be fatal.
By contrast, the fastest skiers in Olympic events only approach these speeds, usually topping off at 70 to 85 miles per hour, but there was a demonstration sport in the Olympics a few years back called speed skiing where the speeds exceeding the terminal velocity of a falling human, around 128 miles per hour. The record is 156 miles per hour. As a contrast, I have skied with Howie Blietz' son, Kyle, who is a fabulous skier and former member of the University of Colorado freestyle ski team and he has a program on his phone that keeps track of one's skiing statistics. I think he said that one set of runs we averaged around 45 miles per hour, which included stops and stuff, so our top speed was probably 50 to 60 most of the time. It feels pretty fast when you go like that.

Join the Economic Development Council on Wednesday, February 24, 2010 as we continue our monthly luncheon series with a noon time presentation by LaTrice Porter-Thomas, Environmental Quality Manager, Cook County Department of Public Health. Ms. Porter-Thomas will present, "What is Bad Air Costing Us?"
Latrice Porter-Thomas graduated from Purdue University with a bachelor's degree in Environmental Health, majoring in Industrial Hygiene and began working at the Cook County Department of Public Health (CCDPH). She left CCDPH after 3 years for the private sector and she returned to CCDPH three years later to manage the newly created Indoor Air Quality (IAQ)/Toxicology Program. Over the years, this program has been instrumental in educating the residents of Cook County and municipal staff on such IAQ issues as, environmental tobacco smoke, asthma, radon and mold, to name a few.
In 2002, West Nile Virus (WNV) surfaced and proved to be a significant public health threat in Cook County. LaTrice was assigned the lead in monitoring avian and mosquito activity throughout the County in order to gauge human risk. She was promoted to her current position as Environmental Quality Manager. In addition to her above duties, this position also included supervision of the Childhood Lead Poisoning Prevention Program.
The luncheon is at noon at the Tower Club, 39th floor, 20 North Wacker Drive, Chicago. The cost of the lunch and program is $40 for EDC members and $65 for non-members. There is no charge for EDC Corporate and Sustaining Members.

ABOUT THE PROGRAM
TimeLine Theatre is the 2009 winner of the Association for Strategic Planning's (ASP) prestigious Richard Goodman Award for Excellence in Strategic Planning (not-for-profit category). ASP Chicago Board member John Sterling will present a case study exploring how this small, non-profit theatre used strategic planning to guide its development throughout the 2000's and achieve dramatic growth.
In its 2002/03 season, TimeLine Theatre had 107 subscribers and a budget of $125,000. In the current season (2009/10) TimeLine has 2,009 subscribers (at which point the theatre cut off sales) and it is on track to achieve over $1 million in earned and donated income. Thus, in seven years, TimeLine has expanded its subscriber base 20-fold and its income has grown at an average rate of 35% annually. During that same period TimeLine:
· Has won over 25 Joseph Jefferson Awards (Chicago's version of the Tony Award)
· Has transitioned to equity theatre status (i.e., operating in accord with Actors' Equity contracts and pay scales)
· Has hired a professional staff of five full time (and several part time) people - from a starting point of no paid employees
TimeLine Theatre operates with a one page strategic plan. Mr. Sterling will talk about both the planning process TimeLine employs and the core intellectual framework that drives its strategy.
Special Program Bonus: Introduced at our January programming meeting, the ASP Chicago will kick off our March program with a segment entitled, "1-1-15". The purpose of the presentation is to introduce and inform attendees about the ASP/SMA Certificate in Strategic Management (CSM) program.

inform entrepreneurs. MEF meets the second Monday of March, April,October
and November at the Downtown Campus of the Illinois Institute of Technology,
565 W. Adams Street, Chicago, from 6 p.m. to 8 p.m. A suburban meeting is
held the last Tuesday of each month (except July and August) from 7 p.m. to
9 p.m. at the Daniel L. and Ada F. Rice Campus of the Illinois Institute
of Technology, 201 East Loop Road, Wheaton, IL (directions listed). Each MEF
meeting normally features a case presentation/panel discussion that
spotlights a single company in a critical stage of development. We also
feature speakers that bring timely knowledge of current solutions that may
make a difference in how you run your business.
MEF membership is $60 per year or $20.00 to attend a single meeting to see
if membership is something you think would benefit you. If you decide you
want to become a member bring your receipt for $20.00 within 30 days to the
next meeting and you will only be required to pay an additional $40.00 to
become a member for the year. Year is from your payment to the next year.
Students with a valid ID pay $5.00. IIT students with a valid id get in
free.
