Briefly noted, by Ron May

* I got a good scoop yesterday while I was in the ER at Northwestern. The person who called me is a reliable source who tells me he has it from a reliable source. The up and coming firm Total Attorneys has cut north of 50 employees as of earlier this week!
http://www.totalattorneys.com/?GCID=S24000x001&KEYWORD=total+attorneys

I looked on the net for confirmation and found this layoff tracker that seems to focus on law firms which has a lot of good info. http://lawshucks.com/layoff-tracker/

I reported a few weeks ago about the ethics probe that has been dropped against Total Attorneys and the on-going lawsuit. That could have easily raised their burn rate and cut into funds needed for growth.

I am reporting this as a strong rumor but be warned, it is not confirmed.

* Advanced Equities has removed any mention of its London, New York and San
Francisco offices from its website. Not quite sure why, as the old phone
numbers still work.

In unrelated news, the firm was hit with a sexual harassment suit just before
Christmas, by a recently-fired female staffer. The boss she claims harassed
her no longer works at Advanced Equities.

* Barry Moltz has made it into a recent edition of the AARP newsletter.

http://bulletin.aarp.org/yourmoney/personalfinance/articles/is_the_recession_over_.1.html

'I'll Know the Recession Is Over When ... '

When I stop bringing my own drink and candy to the movies."-Barry Moltz, author, motivational speaker, Chicago.

* I hear that Vibes Media http://www.vibesmedia.com/ has had a significant drop off in web traffic in the last year according to Compete.com.

* In my list yesterday I should have included a few more pre-conditions before I die.

Don't worry, I am not planning to die anytime soon. I won't buy the farm until:

16. Robin Cook, Brian Connolly and David Dalka get regular jobs.
17. Jim Eiden stops counting trucks on the highway or working in salvage.
18. Frank Gruber stops looking for people who he hopes are looking at him.
19. Eric Olson holds one job for five years.
20. Julian Pretto is no longer a legend in his own mind.
21. Darcy Evon becomes relevant again.
22. Brad Spirrison writes a column not inspired by a press release.
23. Tom Churchwell smiles.
23. David Weinstein enters a room without an entourage of "yes men."
24. When Todd Allen realizes that geeks don't get laid.
25. Chris Rollyson stops consulting on the topic du jour.
26. When Neil Kane and Nik Rokop find a new a** to kiss other thsn Bob Geras'
27. When Ron Kirschner combs his hair.
28. When Gary Slack finally admits that he really has only ten full-time employees.
29. When I attend a meeting where Sheldon Rosenfield, Chris Rollyson and Spencer Maus are present and none of them asks a question.
30. When Tom Figel stops being a nice guy.
31. When Jason Jacobsohn shows up at a meeting and somebody notices.
32. When Len Bland yells at somebody.
33. When Rachel Kaberon consents to be my wife.
34. When Nancy Munro stops volunteering.
35. When Jason Rexilius stops traveling around the country.

* My trip to the ER yesterday was not a "waste of time" per se but it was an example of one problem in our healthcare system. ER is all about urgent life and death problems. "Concerns" are not enough. I was alarmed by the dropping hemoglobin over 17 days. That certainly contributes to my anemia and my constant sleepiness.

I did find out in the ER that they don't want to do transfusions above 7.0 hemoglobin. A few years ago at St. Joseph's Hospital , they did a transfusion for me below 8.0.

As it turned out, the lab done at the ER showed my hemoglobin to be at 8.9, not .7.8. They buy the 8.9, not the four readings we've had since 1/2/10 showing a steady decline from 9.5. That is because NMH believes in its own labs over the labs of other places.

They did check for internal bleeding and did a test for blood in the stool. It came up negative. They also did an X-ray which was clean and we checked out the oxygen saturation situation.

Bottom line. If my hemoglobin at the ER had been 7.8 or lower, I would be in the hospital right now -- or not??

My sense is that no, I would not be admitted on that basis alone but it would have been a closer call, and the rest of my problems don't excite the folks in ER. Now, if I had a fever of 102 degrees, that might be a different story.

My regular low grade fevers without ideation or fevers without a known origin as they called it are of no real interest to the ER folks. There is no obvious infection so they don't focus on it.

Here is my correspondence with Dr. Cremer, my kidney doc.

They called me and I told them that the hemoglobin itself was not a reason to go to ER. Did they suggest you go to ER?
It is hard to know what to make of the change in hemoglobin. I sure hope it indicates an actual improvement, but also since they use different labs and different calibrations that could also explain it. I have ordered weekly hemoglobins on you for now, so we will need to look at the trend.
You are not supposed to need oxygen unless your oxygen saturation goes below 88%. Maybe it was all a transient process that is going away. Regardless, we need to make sure. Let me know what the ID specialist says today.

Dr. Cremer,

As you know, I went to the ER when they suggested that I do so at the clinic. Aurelia seemed very concerned about the 7.8 Hemoglobin reading and so did Liza. I got there about 3pm and got home a little before 9pm.

As you may also know, the lab test done at the ER showed my hemoglobin to be 8.9.

So, we have four labs from the clinic since 1/2/10 showing a decline from 9.5 to 8.3 on 1/9/10 to 8.1 on 1/16 to 7.8 on 1/19.

Then on 1/21 at Northwestern, we have a reading of 8.9.

What explains this?

Are all the lab readings accurate?

If that is so, it is good news to see the hemoglobin going up, and it means it jumped up 1.1 in two days.

The other thing that came out of my ER visit, at my insistence, I might add, was that we looked into the oxygen saturation situation more closely. We took me off the oxygen and for more than ten minutes while I was sitting there talking to the doctor, the saturation level was 99% or 98%, so it seems that I may not need oxygen for passive activity.

But I wanted to test it under stress conditions so I took a walk down the hall and back. I did feel some strain in walking at about the half way mark, but I came back into the room and checked the saturation which was in the low 90s.

I was somewhat winded but it was not really bad at all which raises the question: Do I really need oxygen on a 24x7 basis?

I slept with the oxygen last night but have taken myself off of it now as I sit at the computer.

I just hope the 8.9 is real and that it continues. You have mentioned epogen resistance and there are ways to deal with that from what I can see on the net.
http://grande.nal.usda.gov/ibids/index.php?mode2=detail&origin=ibids_references&therow=391400

What do you think of using Carnitor aka Levocarnitine?

http://www.rxlist.com/carnitor-injection-drug.htm

Finally, given the way NMH works, there is no practical alternative to using the ER as a substitute for things that could and should be done on an outpatient basis.

I have my appointment with Dr. Angarone, an infectious disease guy, today at 3:20pm.

Ron
+++++++++++++++++++++++++++++++++

May again. I am glad to be home and as I sit here, I am off the oxygen and sm OK, but when I was walking around, I felt the strain.

Ron,

I've always liked you because you did things your way and "called them as you saw them" for better or worse. I'm a bigger fan knowing you're a Die Hard fan.

One of my favorite exchanges from the most recent Live Free or Die Hard:

John McClane: You know what you get for being a hero? Nothin'. You get shot at. You get a little pat on the back, blah, blah, blah, attaboy. You get divorced. Your wife can't remember your last name. Your kids don't want to talk to you. You get to eat a lot of meals by yourself. Trust me, kid, nobody wants to be that guy.
Matt Farrell: Then why you doing this?
John McClane: Because there's nobody else to do it right now, that's why. Believe me, if there were somebody else to do it, I'd let them do it, but there's not. So we're doing it.
Matt Farrell: Ah. That's what makes you that guy

Sort of the independent journalist or start up entrepreneur's creed.

"I won't buy the farm until ... Jeff Meredith gives up his propensity
to comment on politics." - Ron May

I guess you'll be living for a long time then.

I don't plan to stop.

What have I ever been wrong about? I've won every single debate I've
ever engaged in. Of course, consider the competition. Facing off
against Darrell Dvorak is like facing the Washington Generals in a
basketball game ... you can't lose against him.

Speaking of Darrell, here's more information about overstated oil reserves:
http://www.guardian.co.uk/environment/2009/nov/09/peak-oil-international-energy-agency
========================
The world is much closer to running out of oil than official estimates
admit, according to a whistleblower at the International Energy Agency
who claims it has been deliberately underplaying a looming shortage
for fear of triggering panic buying.

The senior official claims the US has played an influential role in
encouraging the watchdog to underplay the rate of decline from
existing oil fields while overplaying the chances of finding new
reserves.
=======================

It's a troubling pattern, isn't it? I know Republicans generally hate
whistleblowers and just dismiss them as "crazy" or "disgruntled" ...

As Darrell touted new finds, I also submitted a damning article from
the Financial Times titled "Oil strikes not enough to quench demand":
http://www.ft.com/cms/s/0/d4253e2c-a3ea-11de-9fed-00144feabdc0.html?nclick_check=1
=======================
One thing all the analysts stress is that delaying an oil supply
crunch will need more than just a slew of discoveries. "If action is
not taken on the demand side, you will not shift it," said Ms Hittle.

It will be up to governments whether the demand-side effect comes from
policies promoting efficiency and oil alternatives or the more painful
demand erosion that comes from economies screeching to a halt because
oil supply again fails to keep up with demand.
=======================

This is why oil alternatives are important!

Btw, a guy who knows a thing or two about energy has just retired --
the great Arthur Rosenfeld has left the California Energy Commission.

Salare Security Presents a Free Webinar On the Subject of VoIP Security:

Hear the same powerful compelling talk that has been delivered to the US Secret Service Electronic Crimes Task Force, the Financial Services Technical Consortium, and the Asset Protection Executives Roundtable and many other IT security industry organizations. Paul Sand will discuss how the introduction of Voice over Internet Protocol (VoIP) substantially changes the threat landscape for an organization. In effect, VoIP has converted a very trusted and reliable service, voice communication, into a less reliable service that is delivered on an infrastructure that must not be trusted. However, the business drivers behind VoIP are so strong and so compelling that they push organizations to absorb this significant additional cyber risk. These risks of VoIP will be presented along with potential mitigation and risk management strategies.

In an ongoing effort to provide our members with the tools they need to
successfully develop their businesses, we are presenting another educational
presentation, entitled:

Using Virtual Resources to Build a Global Internet Company

Myfitbrain was conceived to help people improve their brains as they age to
help postpone some of the natural effects of aging on the brain. Many of
its original competitors were companies well funded by VCs and government
grants. They also charge healthy fees that keep the software out of the
hands of the general older population.

Myfitbrain set about building a virtual company, utilizing the most cost
effective resources from around the world and implementing a unique business
model to make its software product available for free without the usual
internet banner ads, etc. Taking advantage of Crowdspring, Amazon Cloud,
developers in India, and social networking Myfitbrain was able to go from
concept to live in a short 4 month time period.

At this meeting, Jim Hanekamp the founder will discuss how he utilized these
different resources and what worked and what didn't work so well.

Biography

Jim Hanekamp is the CEO /Founder of Myfitbrain. He began conceiving the
need for a company like Myfitbrain when his mother began showing signs of
Alzheimer's and he wanted to do something to help those in the 40-60 age
group delay the natural effects of aging on the brain. He discovered that
similar to exercising the muscles of your body, by exercising many cognitive
areas of the brain a person can build a cognitive reserve. He set out to
build a company that could offer internet based software to improve a
person's cognitive ability and make it free to everyone without utilizing
the overused traditional internet advertising schemes.

Prior to founding Myfitbrain, Jim performed strategic IT consulting for
Ardor Consulting which he founded. Jim spent 6 years as CIO at Schawk, Inc,
a creative and marketing services company, helping them grow globally from
$200MM to over $500MM. Before Schawk, he worked for various consulting
firms such as Deloitte Consulting where he was a Senior Manger and The
Revere Group where as a VP he started their PeopleSoft division and grew it
to over 100 consultants nationwide. He started his IT career working at
companies such as Allstate Insurance, Kraft Foods, and Household Finance.
